Telehealth nurse home visit

Nurse-Family Partnership (NFP) was a recent recipient of a $10,000 South Dakota Fund grant award from the South Dakota Community Foundation. Headquartered in Denver, they have provided services to 2,500 families in South Dakota since 2000. Three LPNs currently serve 19 South Dakota counties along with one tribal community. In addition, twelve nurses serve over 260 active mothers and their families. Funds will be directed to their Phones for Families program. Free phones are provided through this program allowing new moms to connect to a virtual platform and access health information.

Nurse-Family Partnership (NFP) focuses on a primary target population of low-income women who are pregnant for the first time. NFP partners each mother with a registered nurse early in their pregnancy and provides ongoing nurse visits continuing through the child’s second birthday. Prior to COVID-19, they invested in technological innovations and launched their telehealth services in 2017 developing a new mobile application, Goal Mama positioning them well for what was to come.

“We are proud to partner with the Nurse-Family Partnership”, said Ginger Niemann, SDCF Senior Program Officer. “It is crucial that new moms are not isolated during this pandemic and have access to critical healthcare for their newborns”.

In response to the pandemic, SDCF’s Board of Directors established the Coronavirus Response Fund to directly support the needs of communities and nonprofits affected by COVID-19. This fund addresses immediate needs including access to food, shortage of volunteers, healthcare sector support and human services sector support. Donations are being accepted.
